DoseCanvas is a personal record and organizational tool. It does not diagnose, prescribe, recommend treatment, identify pills, verify that a medication or supplement is appropriate, or create a complete drug-interaction or contraindication check. Using DoseCanvas does not create a doctor-patient, pharmacist-patient, or other professional relationship with DoseCanvas or UtopiaCharleston.
Verify every entry
Camera scanning, optical character recognition, barcode reading, and typed entries can be incomplete or wrong. Curved labels, glare, small print, handwriting, and hidden text can cause errors. Compare every medication name, strength, amount, route, schedule, direction, warning, and refill detail with the current container and instructions from a licensed clinician or pharmacist before relying on a chart or printout.
A QR code, Data Matrix code, or barcode may contain only a product or pharmacy reference. It may not contain the patient’s complete directions. Symbols and user-supplied pill photos are visual references, not pill identification.
Contraindications, interactions, and pill organizers
A contraindication is a clinical situation in which a medicine should not be used because its risk clearly outweighs any potential benefit. It is not the same as every warning, precaution, side effect, or possible interaction. DoseCanvas lets users record these categories and their sources, but it does not determine whether a true contraindication exists or verify clinical advice.
Notes about medicine–medicine, medicine–supplement, allergy, health-condition, food, alcohol, pregnancy, procedure, kidney, liver, or duplicate-ingredient concerns are entered by users and are not automatically verified. An empty warning section does not mean a combination is safe. Notify a doctor or pharmacist about unresolved concerns. Do not start, stop, combine, split, crush, or otherwise change any medication or supplement based only on DoseCanvas.
Some medicines should remain in their original packaging or require special storage. Ask a pharmacist which medicines can be placed in a daily or weekly pill organizer.
Emergencies
Do not use DoseCanvas for an emergency or wait for an app response. In the United States, call 911 for an emergency. For a possible poisoning or medication overdose, contact Poison Help at 1-800-222-1222. Seek immediate professional care when symptoms are serious or worsening.
Printed and shared copies
A printout is a snapshot and may become outdated. Mark changes promptly, replace older copies, and protect copies because they may contain sensitive health information. The person sharing information is responsible for confirming that they have the patient’s permission or other legal authority.
Not a medication order or administration record
A DoseCanvas chart is not a prescription, medication order, dispensing label, or medication-administration record. Do not use it as the only source for giving a dose. If the chart conflicts with the current label or a licensed professional’s instructions, stop and confirm the correct information with the prescriber or pharmacist.
Care Partner status
DoseCanvas is not currently represented as a HIPAA-compliant clinical system, electronic health record, medication-administration record, or substitute for an organization’s required clinical workflow. A provider, pharmacy, or care organization must not use the Care Partner plan with patient information until its agreement, privacy, security, access-control, and compliance review is complete.
